Portugal's SNS 24 Health Line Fields Over Four Million Calls by July, on Course for Its Second-Busiest Year Ever

SNS 24 has already taken more than four million calls this year, up 17%, and is heading for over seven million — its busiest year since the 2021 pandemic peak. More than half of triaged patients are steered away from hospital emergency rooms.

SNS 24, the telephone and digital front door to Portugal's national health service, is on course for its second-busiest year on record. The line has already fielded more than four million calls in the first seven months of 2026 — a 17% rise on the same point last year — and is projected to pass seven million by December, according to the state body that runs it.

"This will certainly be the second biggest year ever, after 2021, when demand was absolutely abnormal," said Luís Goes Pinheiro, president of the SPMS (Serviços Partilhados do Ministério da Saúde — Shared Services of the Ministry of Health), which operates the service. The 2021 peak of almost nine million calls came at the height of the pandemic; last year the line handled 5.8 million.

More than a phone number

Most people still know SNS 24 as the triage line reached on 808 24 24 24, where a nurse takes your symptoms and tells you whether to self-treat, book a GP appointment, wait for a call-back or go straight to an emergency department. Crucially, more than half of the patients triaged this year were steered towards primary care, self-care or a remote consultation rather than to a hospital urgência — the whole point of the service, which is to keep pressure off overstretched emergency rooms.

But the phone is now only one channel. The SNS 24 online portal logs around five million visits a year, its mobile app has been downloaded more than 14 million times, and a teleconsultation service launched at the end of 2025 has already completed over 130,000 appointments. A separate psychological-support line takes roughly 500,000 calls a year. Behind it all sit about 5,000 health professionals, including more than 3,500 nurses.

The AI question

SPMS has begun using AI-assisted symptom pre-screening for respiratory complaints, which it says agrees with the assessing nurse's conclusion 86% of the time. The technology is being framed as a way to speed up triage rather than replace the clinician on the line — a distinction that matters as the health service leans ever harder on the platform. Reported user satisfaction with SNS 24 stands at 92%.

What this means for residents

  • Call before you queue: for anything that is not obviously life-threatening, ringing 808 24 24 24 first can save you hours in an emergency room — and the nurse will tell you the right place to go, which is not always the nearest hospital.
  • Register for the app and portal: both let you access services, request some prescriptions and book teleconsultations without waiting on the phone; they are tied to your utente (SNS user) number.
  • Language matters: the line operates in Portuguese, so if yours is limited, it helps to have a Portuguese-speaking friend on hand or to use the app's structured symptom checker.
  • 112 is still for emergencies: SNS 24 is for advice and non-urgent triage. For chest pain, serious injury, difficulty breathing or any life-threatening situation, dial 112 directly.

The rising call volume is a double-edged number.
