If you live in Portugal with a long-term illness, injury or disability, there is one piece of paper that does more for your finances than any other: the Atestado Médico de Incapacidade Multiusos (Multipurpose Medical Certificate of Incapacity), usually shortened to atestado multiusos or AMIM. It is not a benefit in itself. It is the key that unlocks almost every other disability entitlement in the system — reduced income tax, exemption from vehicle taxes, a disabled parking card, waived health-service fees and a route into social benefits. Get it, and a long list of doors opens. Without it, you are paying full price for everything.

This guide explains what the certificate is, the all-important 60% threshold, how the medical board works, how to apply as a foreign resident, the benefits it triggers and — just as important in 2026 — how long it realistically takes.

What the certificate actually is

The atestado multiusos certifies a person's grau de incapacidade (degree of incapacity) as a percentage, assessed against the Tabela Nacional de Incapacidades (TNI — National Table of Incapacities). The legal framework runs back to Decreto-Lei n.º 202/96, the regime that governs how incapacity is evaluated and certified, updated several times since.

Two things make it powerful. First, it is multiusos — "multipurpose" — so a single certificate is accepted across the tax authority, the customs and vehicle service, the health service, social security and town halls, instead of forcing you to prove your condition separately to each. Second, the percentage it states is what every benefit is pegged to. Most of the meaningful entitlements switch on at a certified incapacity of 60% or more, which is the number to keep in mind throughout.

The 60% threshold — why it matters

Portugal's disability benefits are not graduated smoothly; they cluster around the 60% line. Below it, you may still receive a certificate documenting a lower percentage, which can matter for specific purposes, but the headline fiscal and vehicle benefits generally require a permanent incapacity equal to or greater than 60%. Reaching that figure is therefore the practical goal of the assessment, and it is decided strictly on the medical evidence measured against the TNI — not on how the condition affects your work or daily life in a subjective sense.

How to apply: the junta médica route

The certificate is issued after an assessment by a junta médica de avaliação de incapacidade (medical board for the assessment of incapacity). The process, step by step:

  1. Gather your medical evidence. You will need a relatório médico (medical report) describing the condition, plus the supporting exams, imaging and test results that substantiate it. The stronger and more complete this dossier, the smoother the assessment.
  2. Submit the request. The application is addressed to the president of the board of the ULS (Unidade Local de Saúde — Local Health Unit) covering your area of residence. In most regions you can now start the request online through the SNS / eportugal.gov.pt channels or with the help of your médico de família (family doctor); some areas still accept it in person at the health unit.
  3. Attend the junta médica. You are called to be examined by the board, which reviews your file, may examine you in person, and applies the TNI to set the percentage.
  4. Receive the atestado. The certificate states your degree of incapacity and whether it is permanent or carries a reavaliação (reassessment) date.

The assessment itself is free of charge. The certificate is then yours to present, in copy, to each entity whose benefit you want to claim.

The benefits it unlocks at 60%+

This is where the certificate earns its reputation. With a certified incapacity of 60% or more, you can access:

  • Reduced IRS (income tax). Holders benefit on two fronts: a portion of employment or pension income is excluded from tax, and the ceiling on deductions à coleta is higher than under the general rules. In effect, part of your salary or pension is shielded, and you can deduct more.   • ISV (Imposto sobre Veículos — vehicle tax) exemption. A person with a permanent incapacity of 60% or more (or specific qualifying motor disabilities of the limbs) can claim exemption from the one-off vehicle tax when buying a car, requested through eportugal. This is separate from, and on top of, the ordinary ISV rules on bringing a car into Portugal.

  • Disabled parking card. The cartão de estacionamento para pessoas com mobilidade reduzida is issued by the IMT (Institute for Mobility and Transport) and is valid for 10 years, unless your atestado sets an earlier reassessment date. It is recognised across the EU.
  • Health and social access. The certificate supports exemption from taxas moderadoras (SNS user fees), access to produtos de apoio (assistive products) and entry to social supports such as the Prestação Social para a Inclusão (Social Inclusion Benefit). Validity and reassessment

An atestado may be issued as permanent or with a future reavaliação date if your condition could change. A 2024 reform (Decreto-Lei n.º 15/2024) eased a long-standing trap: a certificate now remains valid past its stated expiry as long as you have filed a request for a new junta médica before it lapses and can show proof of that request. That continuity matters, because — as below — the queues are long enough that an expired certificate could otherwise cost you your benefits for months.

The waiting times — plan around them

The single biggest practical problem in 2026 is the queue. Demand for juntas médicas has far outstripped capacity: average waits have run to roughly 48 weeks for a first certificate and around 56 weeks for a reassessment, with tens of thousands of requests pending nationally. The lesson is simple but important: apply as early as possible, the moment a diagnosis or prognosis is stable enough to assess, rather than waiting until you need a specific benefit. The atestado is not the disability pension

A common confusion: the atestado multiusos certifies a degree of incapacity for tax, vehicle, parking and access purposes. It is not the same as the contributory disability pension, which is assessed separately by Social Security against a contribution record.
