Government Orders a Study of a Building-Only Public-Private Partnership for the New Barcelos-Esposende Hospital, With Care Staying in the SNS
A despacho published on 30 October tells ACSS to prepare a proposal for a possible PPP to design, build, finance and maintain the new Barcelos-Esposende hospital. The private partner would only handle the building; the public ULS keeps management and patient care.
The Government has ordered a study of a public-private partnership (PPP) to design, build, finance and maintain the long-promised new hospital for Barcelos and Esposende, in Braga district, and has ruled out handing its running to a private partner. A despacho (ministerial order) published in the Diário da República on Friday says management of the hospital and all patient care will stay with the public Unidade Local de Saúde de Barcelos/Esposende (ULS, Barcelos/Esposende Local Health Unit).
The order, Despacho n.º 13315/2026, was signed on 26 October by Francisco Pinheiro Catalão, Secretary of State for Health Management. It tells the Administração Central do Sistema de Saúde (ACSS, the Central Administration of the Health System) to prepare a reasoned proposal for studying and launching the partnership under Decreto-Lei n.º 111/2012, the general law on PPPs. That law, the order notes, lets the State pass the risks of designing, building, financing and maintaining a building to a private partner only if the model shows an advantage over other ways of contracting.
The deadlines
- 15 December 2026: the SNS Executive Board must propose the hospital's clinical role: its place in the national health service network, its level of specialisation and how it links with other hospitals. The ULS must hand ACSS its technical and clinical data by the same date.
- 1 March 2027: ACSS must send its reasoned PPP proposal to the Secretary of State.
The order sets no date for a tender, the start of building work or opening. It refers to a "possible" partnership, so the final choice depends on that study.
A building-only partnership
The model differs from Portugal's first wave of hospital PPPs. According to ACSS, the contracts for Cascais, Braga, Vila Franca de Xira and Loures had two strands: one for the building and one for clinical care. The clinical strands in Braga, Vila Franca de Xira and Loures ended between 2019 and 2022 and returned to public management; their building contracts continue. The Hospital de Lisboa Oriental contract signed in February 2024 covers only the infrastructure, which is the model now proposed for Barcelos. The government's wider plans for hospital PPPs were covered in our April report.
Two decades of waiting
The project has been promised for about 20 years. Lusa recalls that the government and Barcelos council signed a protocol for a new hospital in 2007, but it never went ahead. The council bought the land itself: in December 2025 it signed an agreement giving it 156,930 square metres at Quinta de Santa Maria for the hospital and its access roads, part of it bought for 4 million euros.
According to the council, the government has put the investment at 200 million euros. The plan is for a building of more than 25,000 square metres, with 218 inpatient beds, 30 outpatient consulting rooms, six operating theatres plus two in reserve, and MRI and CT scanners. Lusa reports that it will serve more than 150,000 people in the two municipalities.
In a statement on Friday, Mayor Mário Constantino Lopes called the hospital "an absolute priority" and said that what matters now is that "each stage is met".
What it means for patients now
Nothing changes for now. Care continues at the Hospital de Santa Maria Maior, which Lusa reports is housed in a building owned by the Santa Casa da Misericórdia. The mayor, quoted by Lusa, described the current building as having an overcrowded emergency department and too few operating theatres. Under the order, once the new building opens, care there will still be provided by the ULS, inside the SNS.
